Eligibility

All conditions must be current as of 11:59pm, May 12, 2017.

  1. Subscription must be current.
  2. Must be a ‘Lyfe Experience’, ‘Double the Lyfe’ or ‘More to Lyfe’ subscriber.
  3. Must be 21 years old or older.
  4. Cannot be a current employee or contractor of The Lyfe Magazine or Righteous Productions.
  5. Cannot be a member of the board of directors of The Lyfe Magazine or one of their direct family members (husband, wife, mother, father, child(ren) or sibling).

Terms and Conditions

  1. This certificate entitles the recipient to two nights of hotel accommodations at one of the locations described (subject to availability and certain restrictions). To be eligible for this giveaway you must be 21 years of age or older. A husband and wife or cohabitants must travel together to use this certificate. Upon check-in, hotels will require an imprint of a major credit card (NO DEBIT CARDS) to cover the cost of incidental charges and room tax. This offer is only available to U.S. residents or Canadian residents.
  2. To activate your certificate, locate the ‘ACTIVATE NOW’ section on your certificate and choose your option. If you have a U.S. checking account, you can register online, pay the $50 refundable deposit plus any applicable child fees and complete the planning form. Please allow 30 days for processing.
  3. When registering by mail, complete the Validation Form and mail to: Grand Incentives, Inc., 7560 Commerce Court, Sarasota, FL 34243 (Validation Form must be the original form; no copies allowed). Upon receipt of the Validation Form, a Vacation Planning Form will be sent via email. Print and complete your Vacation Planning Form and return to Grand Incentives along with your refundable deposit and any applicable child fees. IMPORTANT: Your $50 refundable room deposit and any applicable child fees must be submitted with your planning form (U.S. funds only) to move forward with the reservation process. Your completed Vacation Planning Form must be received a minimum of 45 days prior to your requested arrival date.
  4. Incomplete or illegible registration forms will not be acknowledged, processed or returned. No replacements will be issued.
  5. Prior to confirming your reservation, you will receive a destination verification email (approximately 30 days in advance of your requested travel date).You will be required to acknowledge this email before your reservation can be booked. Reservations are booked approximately 30 days in advance of your requested arrival date. All reservations are based upon Grand Incentives’ allotment of availability. You will receive a confirmation email once your reservation has been confirmed.
  6. Your confirmation letter and two forms of identification (driver’s license and a major credit card – NO DEBIT CARDS) must be presented upon check-in.
  7. This vacation offer provides standard accommodations for two adults. Up to two children under the age of 18 may stay in their parents’ room for $10 per child, payable to Grand Incentives, Inc. and submitted with your Vacation Planning Form. Sorry no pets allowed.
  8. Travel must be completed by the expiration date.
  9. Check-in date varies depending on destination. When submitting your Vacation Planning Form you will be required to give 3 different location choices (required to process your reservation due to changing availability). All destination have limited availability during peak season or holidays. The Pocanos, Hilton Head, Myrtle Beach, Wisconsin Dells, Andover and Atlantic City have limited accommodations available during June, July and August. For Atlantic City, Reno, Lake Tahoe and Las Vegas, only Sunday through Thursday occupancy applies. All accommodations are in a hotel or motel in the resort area.
  10. This certificate is valid for standard hotel accommodations only and does not include transportation, taxes, meals or any other miscellaneous expenses. At the time of booking, the total financial obligation to traveler will range from $24 to $36 for hotel taxes. Hotel taxes are collected upon confirmation with a credit card. Grand Incentives, Inc., its agents, assignees and designees, act only as agents for hotels and will not assume any responsibility for any loss, delay, accident, injury or damage to persons, property or any liability whatsoever arising from or in conjunction with services provided.
  11. There is a $50 fee to reschedule or change a confirmed reservation. No changes can be made to a confirmed reservation within 14 days of your check-in date. To request changes, call the Grand Incentives, Inc., Help Desk at 941-552-5070, Monday through Friday, 9am to 5pm EST. Trip protection is highly recommended.
  12. If you cancel within 14 days of your check-in date, you will forfeit your $50 room deposit. All cancellations must be submitted in writing.
  13. Two or more vacation certificates may not be used consecutively or within a 12-month period. This offer cannot be used in conjunction with any other promotion. You can request a refund of your deposit by mailing copies of your reservation letter and hotel check-out statement to Grand Incentives, Inc., 7560 Commerce Court, Sarasota FL 34243.
  14. This offer is not valid for group travel or for attending a convention. No exceptions.
  15. This certificate may be transferred to a third party (prior to booking) as a gift offer only, providing the recipient meets the terms and conditions of this certificate.
